I think the best way to create a story in an RPG is to start off by getting a notebook and thinking of interesting places to go in a game. Write down any ideas you have, no matter how ridiculous it is. If you think of having characters go underwater, write it down. If you think of having a character get stuck in the insect world, FOR THE LOVE OF GOD write it down. It doesn't matter if it's crazy right now. Just write down ideas.
Then, comes step two. Think of a villain. Once again, write down any ideas (on a different page) of what he/she/it can do, no matter how ridiculous.
Step three. Think of a hero. See above.
Step four. Think of supporting cast. See above.
Step five. Now is the difficult step. Now, you must start thinking of a plot that can connect the areas/events. First off, get rid of any stuff you think is too crazy. Second, look at the villain's doings, and think of a motive and goal. Once you've done that, move on to connecting the areas. Think of the events that will move the heroes from place to place.
Step six. Once you've connected everything, make the ending. Look at the events and figure out how to make closure to them.
Step seven. Look at your entire setup. Are there any plot holes? If so, figure out a good way to solve them.
Step eight. Write up the script to your game. Just the dialogue in the events, not the musings of random townspeople (unless they have something important to say.)

That's a good method, I suppose. That info can be helpful for someone with not many ideas.
